Seeking your feedback

The Alzheimer’s Disease Program (ADP) of the Texas Department of State Health Services is conducting a survey to learn more about current and former unpaid caregivers’ experiences. Your responses will contribute to helping other Texas caregivers as they tend to their loved ones and themselves.

 

Who should participate?

To be eligible to participate in this survey, you and/or care recipient must have lived in Texas in the past 12 months.

How long will the survey take?
The survey should take no longer than 15 minutes to complete.

How will my responses be used?
Your responses will remain anonymous. A summary of the results will be used by the ADP and the Partnership to better understand the needs of unpaid caregivers and progress made toward the strategies and activities listed in the Texas State Plan for Alzheimer’s Disease 2019-2023 (state plan).

Please complete the survey by Friday, January 29th at 5:00 PM.

Please feel free to share this survey with family, friends, and/or community members who might know of someone who has provided unpaid care or assistance to a relative or friend with Alzheimer’s disease or other dementia in the past 12 months.
