Event Details:
This session is designed for caregivers and family members to discuss the importance of making a plan for the future, as well as some of the decisions you will need to make related to care, including advance care planning. As a participant, you can share and reflect with others on their experiences while also discussing strategies to help your loved one prepare for the next steps.

Objectives:

  • Understand different areas where planning ahead will be important
  • Differentiate transitions in care that will have important implications for planning
  • Identify community resources that can assist with planning
  • Describe the important role of palliative care in supporting caregivers; discuss advance care planning.

About the Speaker(s)

Jennifer Dear is a highly experienced healthcare professional with over 29 years of diverse nursing experience. She specializes in pain management, hospice, palliative care, long-term care, and Alzheimer's care. Throughout her career, she has held pivotal roles such as Director of Nurses, Assistant Director of Nurses, and Senior Hospice Sales Representative, showcasing exceptional leadership skills. Jennifer is recognized nationally with multiple Pinnacle Leadership awards, and she is committed to pursuing excellence in her work. Currently, she is pursuing a Master of Science in Nursing, demonstrating her dedication to growth and advancement. Beyond her work, Jennifer actively engages in community outreach and advocacy. She has a deep commitment to holistic care and education, empowering individuals and families to navigate healthcare efficiently.

Claudia Diaz currently holds the position of Director of Community Advocacy for United Healthcare Community and State. She has resided in Austin for the past 17 years but has recently relocated to San Antonio. After completing her education in Communications at South Texas College, she began her professional career in the non-profit sector. She has always been passionate about volunteering and meeting new people and enjoys singing, hiking, and traveling in her free time. With 22 years of experience in healthcare management, Claudia has devoted her career to providing compassionate care to patients and community members in need. Over the years, she has developed a deep understanding of the complex healthcare industry, including regulatory compliance, patient care management, and financial analysis.
